Payment is due at time of services being rendered. Payment is taken the morning of your session.
A credit card is required to be on file to cover co-payments, outstanding balances and / or no shows / no calls.
"No Surprises" Billing Policy
Notice to clients and prospective clients:
Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Please note: when insurance is billed we are required to provide a diagnosis. Also, you are responsible for the co-pay and any deductibles at the time of your visit. A credit card will be kept on file and billed at time of service.
Although we do not participate with all managed care insurance plans at this time, you may be able to collect out-of-network benefits for our sessions. We will furnish you with a statement of services and the necessary diagnosis and codes if you would like to submit your claim for possible reimbursement.
Questions to ask your insurance company about out-of-network benefits
Therapy may be covered in part or in full by your health insurance plan. Check your coverage carefully by asking your provider these questions:
Do I have “out-of-network” mental health insurance benefits?
What is my deductible and has it been met?
What is my “co-insurance”?
How many sessions per year does my health insurance cover?
What is the coverage amount per session?
Is approval required from my primary care doctor?
This online form is a convenient way to schedule appointments; however, please contact us by phone / email if you need any assistance. New clients: Documents will be sent via the TherapyNotes Portal for completion prior to your appointment.
Cancellations require 24 hours notice, or you will be billed a missed session fee. We have reserved a specific time to meet with you, so please call 281-940-5820 or email email@example.com for cancellations or changes.
The charge for same day cancellations or no show is the full self pay cost ($150 and up).
We realize life happens and sometimes events occur out of our control, but as a courtesy please provide advanced notice if possible so someone else can schedule an appointment during that time.
